I left Argyll Medical Group in 2008 and am now a hospitalist at Enloe Hospital in Chico. I've taken down most of this website except for the bare minimum as I find I rarely get record requests given the amount of time that has passed since I closed my outpatient practice.

Requests for Records:
There is no charge for records sent directly to a new provider--and you may simply fill out a release of information request at your new provider's office. Have your new provider mail a release of information request to: Kevin Brown M.D. 702 Mangrove Ave., #341 Chico, CA 95926 .

If you desire a personal copy of your records you may send a release of information form to the address below and a CD copy readable with the Adobe reader will be mailed to you. There is a $20 charge per individual. If you do not have the Adobe Acrobat reader to open the release of information file you may download it from the web by clicking on its Icon to the left of this column.
