My outpatient clincal practice closed April 11,
2008 There is no new information since this page last updated 5/27/09 I left Argyll and am now a hospitalist at Enloe Hospital in Chico. See the link to your left titled "Information for First Time Visitors" for more information. My practice has been closed now for over a year. There is a physician tentatively scheduled to open up a practice in my previously occupied space in October 2009 For the most up to date information about other local doctors who may be able to accept you as a patient go to the frequently asked questions page. You may also check with the Butte-Glenn Medical Society to see if they have further information. Their address is 85 Declaration Drive Ste 200 Chico, CA 95973. Their phone number is (530) 342-4296. In general I've found my web site to have information that is more up to date than the information from the medical society. Requests for Records: A release of information form to have your records sent to a provider outside Argyll Medical Group is available here . If you do not have the Adobe Acrobat reader to open the release of information file you may download it from the web by clicking on its Icon to the left of this column. Do not send record requests to my office at Argyll Medical Group as I will not be maintaining office staff there to process those requests. There is no charge for records sent directly to a new provider--and you may simply fill out a release of information request at your new provider's office. If you desire a personal copy of your records you may send a release of information form to the address below and a copy will be mailed to you. There is a $20 charge per individual ($23 if you request it from the Argyll office). Records come on a CD rom readable with the Adobe reader.
